Do you know of a diet and/or exercise regimen that might result in reducing breast size in a woman who's 25, has always been thin no matter what she ate (approx. 5'4" 100-105 lbs.) but has always had relatively large breasts? This is probably her last hope before a breast reduction surgery.

You see, some women really have breasts that are too big for them, it is not funny for them. Their back does hurt... try carrying a watermelon on your chest all day...

If this woman is a candidate for breast reduction surgery (that is, if a reputable doctor would operate on her), she probably has heavy enough breasts to cause her significant discomfort. At 5'4" and
100-105 lbs., this woman is extremely slight. Large breasts might well be placing a strain on her back, for example.
And to Ann: It's unlikely that diet or exercise would help in this case. This woman clearly has very low bodyfat. The only way to reduce breast size is overall weight loss, and it won't come only from the breasts. And weight loss wouldn't be a good idea for someone this light.

Within limits, the question isn't what you're carrying; it's how you're carrying it.
Back pain is often caused by having insufficient strength in your abdominal muscles -- the transversus abdominus and the obliques. Most aerobic classes emphasize the rectus abdominus -- a muscle that has far little impact on alleviating back pain. Pilates is the one thing I know which is most effective at developing these and other postural muscles.
I suggest having a look at _The Pilates Promise_ at your local bookstore and look at the before/after pictures after the 3 women worked with the instructor for 10 weeks. While none of them have comparable measurements to the OP's friend, you can definitely get a good sense what's available.
Any surgery carries risk; I strongly recommend trying Pilates before seriously considering such an option.
